NHS Patient Satisfaction Survey Template

This NHS Patient Satisfaction Survey Template is used to gather feedback from patients about their experiences with healthcare services in England. The survey covers aspects such as the quality of care, access to care, communication, and overall experience.

Patients are asked to rate their satisfaction on a scale of 1 to 10, and demographic information such as age and gender is also collected.

The survey also includes an open-ended question for additional comments or feedback. Results are used to identify areas for improvement and to inform NHS policies and practices.
