What is the purpose of this survey?
Healthwatch Hounslow is conducting a survey with service users of the Healthy Hounslow services. The purpose of the survey is to learn about your experiences as a client and obtain feedback on your journey through the service. This will help us to understand how the services are working and make recommendations for improvement based on your feedback. The feedback we collect will be analysed anonymously, and a report produced highlighting the key themes and trends and outlining recommendations for improvement to Healthy Hounslow’s programs.
Please complete this survey if you are a client of Healthy Hounslow. You will have the choice of completing this survey on paper, or if you prefer, we can arrange a telephone or online call for you to discuss these questions with a member of our team.
