Let's talk about dying - your views

0%

Everyone has different hopes, fears and wishes about what happens when they die. Thinking and talking about this can be difficult - but it can also be helpful.

Healthwatch Oxfordshire would like to hear your thoughts about what is important to you towards the end of your life, and what might help you to talk about your wishes with family, friends or health and care professionals. We also want to hear about any experiences you have had of end-of-life or palliative care services.

We will share what you tell us with people and organisations who provide services or make decisions about end-of-life care locally, to help improve the support for people and their loved ones.

Who are Healthwatch Oxfordshire?
Healthwatch Oxfordshire is an independent charity for people who use health and social care services. We find out what matters to local people and make sure their views are heard by the people who make decisions about those services. We will use what you tell us to help shape support and improve services.

This survey is anonymous - you do not have to say who you are and we will not identify you.
