Prominate is committed to ensuring a high standard of ethical and environmental trade practices, including the provision of safe working conditions and the protection of workers’ rights, across its global businesses.

We require conditions at factories and work sites to be checked, to ensure compliance with our Code of Ethical Policy - The Ethical Trade Initiative (ETI) Base Code and The Ten Principles of the UN Global Compact.

In many cases we require factory assessments and audits of work sites and labour standards. However, to begin with, this Supplier Self-Assessment Questionnaire (SSAQ) is a tool to help identify such cases. Self-assessments are a valuable tool in any supply chain sustainability program. In answering these questions we appreciate your openness. The information used in this SSAQ is treated in confidence.

Please answer all the questions in the SSAQ truthfully and accurately. Once completed, we will inform you of any further steps. If you have any questions about the SSAQ do let us know by emailing us at SSAQ@prominate.com.
