Personalised care means that people have a choice and control over the way that their maternity care is planned and received. This is based on what matters to that person, what that person needs and what that person prefers. 

Informed decision making means that anyone who is receiving care, have been fully supported and informed about all options surrounding their care and treatment. Personalised care and support planning identifies what matters to each person using maternity services and makes sure that their care reflects this. 

The purpose of this survey is to identify whether you received personalised care throughout your maternity experience, this helps to highlight areas of good practice and support any service improvement needed within individual Hospital Trusts and across Lancashire and South Cumbria.

 

All responses will be anonymous. All information given will be held by Healthwatch Lancashire and remain secure and confidential, and will be used in line with the regulations of the Data Protection Act 2018. We will not pass your details on to any third parties, and it will not be used for marketing purposes.
