The following survey aims to look at improving services and care for families that access the neonatal care within Hertfordshire and West Essex, this includes the Lister Hospital, Watford General Hospital and The Princess Alexandra Hospital. All of the units are Local Neonatal Units. Please share your experiences with us.

Why we need your information
The data on this form is being gathered for the purpose of improving neonatal services and care. Hertfordshire and West Essex Local Maternity and Neonatal System will use this information to help shape the service for future women and birthing people, their families and babies.

What we will do with your information
The information you give us will be held by the Hertfordshire and West Essex Local Maternity and Neonatal System and will be used for the purpose of improving neonatal services and care. In order to do so, we may share your information with the three hospital Trusts (East and North Hertfordshire NHS Trust, The Princess Alexandra NHS Hospital Trust and West Hertfordshire NHS Hospital Trust) and Hertfordshire County Council who together form the Hertfordshire and West Essex Local Maternity and Neonatal System.
How long we will keep your information
The information that you supply to us will be kept on file for three years.
