About this survey
This survey is for people who have given birth in the last 12 months under the care of Royal Surrey County Hospital and Surrey Hills Home Birth Team maternity services. It should take about 5-10 minutes to complete. There are 39 questions, please answer as many as you can. Your thoughts and experience are important and can make a difference to local maternity care.
If you gave birth more than 12 months ago - your thoughts are still very important to us. To share your feed back please email us at rsch.mvp@gmail.com . Your email will be treated in confidence and your details kept anonymous unless you agree otherwise. You can also make an official complaint via PALS.
How we will use your feedback
The information in this survey is being collected by the Maternity & Neonatal Voices Partnership (MVP). We are not employed by the Royal Surrey in fact many of us are volunteers. We work with the Royal Surrey to help improve maternity and neonatal services for people like you.
Your feedback will be used to identify themes for improving local maternity and neonatal services, and celebrating and encouraging good practice. No specific action will be taken in relation to your individual care. If you would like to make more detailed comments or raise a specific issue about your care please email rsch.mnvp@gmail.com. Please also get in touch if you would prefer to speak to us in person by video call or telephone.
We will only use your data for this purpose, we never share personal data with other organisations, and we will delete all personal data, if any, once we finish the reporting of our findings from this survey. It is your decision whether you decide to use the free text boxes to include additional information (including identifiable or personal data) which is beyond the data that we are collecting within this survey.
