What is the survey about?
We are interested in hearing about your experiences of your local Admiral Nurse clinic service.

The survey should take around 5 minutes to complete – depending on how much you would like to say.

Do I have to fill in this survey?
You do not have to fill in this survey if you do not want to – it is up to you to decide whether or not to take part.  If you decide to take part, your answers will be treated in the strictest confidence. If you do not want to take part, you do not need to do anything further.

How will my responses be stored and used?
Dementia UK’s Insights & Evaluation team will collate and analyse the responses to this online survey.
 
The results will be included in a report for the local Admiral Nurse clinic service and will help demonstrate the impact of the service and how it can improve. In the report we may include quotes/comments from the open-ended questions in the online survey. We will ask you, in the survey, how you would like these to be attributed (if your responses are chosen). Quotes are generally attributed to either ‘professional stakeholder’ or by job title.
 
Dementia UK may also use the results in other publications e.g. reports, conferences, presentations, journals or through the Dementia UK website.

Data from the survey will be securely stored by Dementia UK, in accordance with our privacy notice.  Any information you provide is completely voluntary.

You can stop completing the survey at any point - any responses you have completed up to this point will be automatically saved and may be included in our analysis.
