Survey on Hospital Discharge for Staffordshire Residents

We want Staffordshire people to tell Healthwatch about their experiences of hospital discharge (leaving hospital).

The information we collect will be anonymous. The feedback will be analysed and published in a report. We may use quotes in our reports but we will not use any information that will identify you. The report will be shared with organisations who run and commission these services in order to understand any problems and how the service could be improved.  If you are sharing information on behalf of another person, make sure that you have their permission to do so.

We will also ask you to volunteer some personal information to understand how different groups experience local health and social care services.  This supports our focus on improving equality, diversity and inclusion.
