DHSC Leadership Programme nomination survey

Instructions
The purpose of this survey is for Directors of Adult Services or other appropriate senior leaders to register nominees for the DHSC Leadership Programme (DHSC LP). The survey will ask for your organisation information and nominee's contact details.

You will be able to nominate four participants for the programme. However it is likely only one of your nominees will be offered a place.

Please list your nominees in order of priority.

Nominees will be from one of the four following groups:

• AMHP lead
• Principal Occupational Therapist
• Principal Social Worker (Adults)
• An aspiring leader who is from a Black & Global Majority community and who aspires to be an AMHP lead, POT or PSW

Places will be allocated on a first come first served basis and not all nominees are guaranteed a place. 

We need to ensure an equal spread of professional groups and aspiring leaders, therefore first choices are not guaranteed a place. 

Anyone not offered a place will be put on a waiting list.

As Data Processors we will record this information in our database and contact each person directly for the purpose of course delivery for the DHSC LP. By providing the information below you will be acting as Data Controller and confirm that you have sought explicit permission to share this personal information with us.
