Co-Designers Network Expression of Interest

Thank you for registering your interest to be part of the Co-Designers Network!
As part of the Co-Design Network, you will be sent a Co-Design Kit. For this kit to be sent to you we will require your email address, full name and home address. By signing up to be part of the Co-Designers Network, you consent for this information to be held by Lab4Living at Sheffield Hallam University. You can also opt-in to giving other demographic information which will help us understand the make up of the network. This information, however, is not mandatory.

By being part of the Co-Design stage of the ORIGIN Research, you won’t be able to participate in any other opportunities from this project such as the final study. If you’d like to be part of those work packages or have any questions about this, give us an email and we can discuss it further with you! 

All data collected in this form will be stored in a GDPR compliant manner, only available to the immediate study team. By ticking "yes" you consent to the data collected being used to inform the overall project.  *
