Welcome to the survey start page. Ofsted and the Care Quality Commission (CQC) are visiting your local area to understand more about preparation for adulthood arrangements. Thank you for helping us by answering this survey.

As part of the visits, inspectors want to know more about how children have been supported to prepare for adulthood. To support inspectors to understand this, this survey asks questions about the 4 key pathways for preparation for adulthood, based on the SEND code of practice: employment, independent living, community inclusion, and health.  
 
This optional survey should take around 5-10 minutes to complete. Your answers will not contribute to any judgements in your local area, and we are not asking for any details that may identify you.  


 


This survey is for parents, carers and professionals only. The survey for children and young people over 11 with SEND who are responding about their experiences of being prepared for adulthood is here.


We will use the evidence we collect through surveys primarily to inform our final national report. Findings will not be attributed to any specific area. We will also review survey responses periodically.  

 
If you are both a parent/carer as well as a professional, please select one perspective for your response. You are welcome to retake the survey from the alternative perspective.

SmartSurvey will pass your information to Ofsted, and Ofsted will delete the information 2 years after the final national report is published.

Safeguarding concerns
Our surveys are anonymous, but if what you tell us makes us worried about you or another child or young person is at risk of harm, we will tell the local authority.
