Why are we carrying out this survey?
Type 2 Diabetes is a life-changing condition but can be well managed through engagement with a range of diabetes support services.
To help people use these services, NHS South Yorkshire Integrated Care Board (ICB) is looking to improve the way support for people living with Type 2 Diabetes is promoted and delivered. The results from this survey will help us understand people's experiences of living with Type 2 Diabetes and how to give them more information on the support available.
This survey is being conducted by Social Change, an independent social research and marketing company, on behalf of the South Yorkshire NHS ICB. It should take you around 10 minutes to complete, and an answer is not required for all questions. Those with an asterisk (*) need to be completed so you can progress with the survey.
