Introduction

 

Do you live in South Yorkshire, are over 16 years of age, and have Type 2 Diabetes? We want to hear from you and know the best way to promote information relating to Type 2 Diabetes.

Why are we carrying out this survey?

Type 2 Diabetes is a life-changing condition but can be well managed through engagement with a range of diabetes support services.

To help people use these services, NHS South Yorkshire Integrated Care Board (ICB) is looking to improve the way support for people living with Type 2 Diabetes is promoted and delivered. The results from this survey will help us understand people's experiences of living with Type 2 Diabetes and how to give them more information on the support available.
 
This survey is being conducted by Social Change, an independent social research and marketing company, on behalf of the South Yorkshire NHS ICB. It should take you around 10 minutes to complete, and an answer is not required for all questions. Those with an asterisk (*) need to be completed so you can progress with the survey. As a thank you for your time, you will be given the opportunity to enter a prize draw at the end of this survey to win a £100 voucher from One4All.
