We know it is important that services for children and young people with special educational needs and/or disabilities (SEND) and their families are improving all the time. We want to hear your views to make sure that education, health care and social care are as good as they can be.
When you start the survey, you will find three different routes taking you to three sets of questions:
• One for parents and carers with a child or young person with SEND
• One for children and young people with SEND aged 11 to 25
• One for professionals who work with children and young people with SEND and their families

An anonymised summary of the information we receive will be published on the Norfolk SEND Local Offer web pages at
www.norfolk.gov.uk/SEND. No personal details will be shared.
The results of this survey will be used to help us plan how to improve the lives of children and young people with SEND by:
- helping those who work with children, young people and families to understand and make good decisions about SEND services
- informing the Norfolk Area SEND Strategy which is refreshed every year and completely reviewed every four years
- providing useful information for the Ofsted/Care Quality Commission (CQC) Area SEND revisit, expected later this year.
This is the first time we are running this survey and we plan to run it every year.
If you need help to complete this survey you can ask a friend, family member or someone else you trust to help you fill this in.
