This form is for schools to complete, using information from parents as far as is possible.It enables schools to tell the LA what other arrangements are to be made for a child's education. It is important that information is made available so that education files can be forwarded to the appropriate school and that all children are tracked from one form of education to the next for safeguarding purposes. State-funded schools need complete this form for pupils leaving an infant school at the end of Year 2, a junior or a primary school at the end of Year 6 or a secondary school at the end of Year 11 ONLY where the destination is unknown.
