Community health services support people in lots of different ways, such as support from district nurses where people live, support from specialist nurses to manage a long term condition, or support to return home after a stay in hospital. They are commonly used by children, people living with long term conditions and older people.
The services take place in lots of different places, including people’s homes, care homes, health centres, children’s centres, and schools. They might sometimes take place at Lewisham hospital or at a GP practice.
At Lewisham and Greenwich NHS Trust, we are working to develop a new plan for these services. This will set out our ambitions for the future.
