Please complete this survey about your new draft Education Health and Care Plan (EHCP) or draft Amended Education Health and Care Plan (EHCP) which you have recently received. This will be your plan if you are over 13, or your child’s plan. You can ask someone else to complete this for you.

There are 13 questions and it should take about 5 minutes to complete. Your answers will help us know what is working well and what could be better. We will make sure we publish the results and what difference the feedback has made. We will publish this on the Local Offer website.
