What is the survey about?
We’d like to hear about your experience of the hospital Admiral Nurse service during the recent hospital admission for the person with dementia you care for.
Who is the survey for?
This survey is for carers who have accessed the hospital Admiral Nurse service. We ask everyone who has received support from the Admiral Nurse service if they would like to complete the survey.
Do I have to fill in this survey?
No, if you don't want to complete this survey you do not have to.
If you would like to tell us about your experience, your answers will be completely anonymous (no personal information that could identify you is requested).
The survey should take around 10 minutes to complete – depending on how much you would like to say.
How will my responses be used and stored?
Dementia UK’s Insights & Evaluation team will collate and analyse the responses to this online survey.
The results will be included in a report for the hospital Admiral Nurse service and will help demonstrate the impact of the service and how it can improve. We may include quotes/comments from responses to the survey in our work, however these will never identify individual respondents.
Dementia UK may also use the non-identifiable results in other publications e.g. reports, conferences, presentations, journals or through the Dementia UK website and social media channels.
Data from the survey will be securely stored by Dementia UK, in accordance with our privacy notice. Any information you provide is completely voluntary.
You can stop completing the survey at any point - any responses you have completed up to this point will be automatically saved and may be included in our analysis.
