The survey will only take 5-10 minutes, depending on how much you would like to tell us, and we do ask that your experience with health and care services has been in the last 12 months.
Please note you can answer “prefer not to say” to any of our questions including health conditions, ethnicity, sexuality, and education. However, this information helps us to understand how different groups experience health services so we can reduce inequalities.
We’ll use your survey responses to understand local experiences and report to the providers of all of the services about what is working and what needs improvement.
