What is the survey about?
The survey is about your experience of referring to the Frailty Consultant Admiral Nurse Service.

Who is the survey for?
This survey is for anyone who has referred someone to the frailty service. It should take no more than 5 minutes to complete. 
 
Do I have to fill in this survey?
You do not have to fill in this survey if you do not want to. It is up to you to decide whether or not to take part.  If you do not want to take part, you do not need to do anything further. If you decide to take part, your answers will be completely anonymous (no personal information that could identify you is requested).
 
How will my responses be used and stored? 
Dementia UK’s Insights & Evaluation team will collate and analyse the responses to this online survey. 
  
Dementia UK may use the non-identifiable results in reports, conferences, presentations, journals or through the Dementia UK website.
 

Data from the survey will be securely stored by Dementia UK, in accordance with our privacy notice.  Any information you provide is completely voluntary.

You can stop completing the survey at any point - any responses you have completed up to this point will be automatically saved and may be included in our analysis.
