We are conducting a survey to learn more about the experiences of people from only African, Caribbean, and mixed heritage backgrounds, who have been diagnosed with type 2 diabetes.

This includes when you were first diagnosed, how you access and use health services, any lifestyle changes you've made, and the outcomes you have experienced.

We're also keen to learn if the services you've used feel culturally appropriate and meet your needs. 

Your feedback will help us improve diabetes care and support for diverse communities.

Please note that your responses to this survey will remain completely anonymous and confidential. We will not collect any personal information that could identify you, and all data will be used solely for research purposes.
