Survey of Children and Young People Emotional Wellbeing and Mental Health Services in Shropshire, Telford and Wrekin
This survey aims to gather feedback from children and young people, and their parents or carers who have used emotional wellbeing and mental health services (CAMHS) in Shropshire, Telford and Wrekin.
Please complete this survey if you have experience of emotional wellbeing and mental health services and are:
- a young person aged over 16 (children under 16 should ask a trusted adult to complete the survey on their behalf).
- a trusted adult helping to facilitate a child or young person to complete the survey.
- a parent or carer working with children and young people.
If you are under the age of 16, please ask a trusted adult to help you complete the survey on your behalf.
This survey builds on previous feedback that has already helped shape services. Your input is critical and will help decide how emotional wellbeing and mental health services are planned and delivered in Shropshire, Telford and Wrekin in the future.
