Cheshire West and Chester Movement and Physical Activity Survey

Partners across Cheshire West and Chester are using this questionnaire to find out about the physical activity levels, attitudes and behaviours of residents, as well as the opportunities that exist to support residents to become more physically active.

The survey should take around 10 minutes to complete and you can quit at any time.

As a thank you for completing the survey, you will be entered into a prize draw to win a three month Brio Leisure passes - there are four passes up for grabs! To enter, you will need to enter your email address at the end of the survey - if you don't wish to enter the prize draw you do not need to provide this information.
