Over the next six months, Healthwatch will be visiting the following five wards and one unit managed by Avon & Wiltshire Mental Health Partnership, talking to patients and listening to their experiences.

We would like to hear your views as the family, friends and carers of patients so that we can highlight the perspective of everybody involved in the care and wellbeing of your loved one. We have used the word "patient" for clarity but we do understand that for you this represents a loved one and/or family member.
This survey has 11 questions with a further 11 optional questions about yourself, and will take about 10 minutes to complete. Please try to answer all questions and be as detailed as you can with your answers. We are going to use your feedback to find out what is working well and where improvement could be made. Each question leaves you room to make further comment on your answer.
