We welcome your personal views as someone involved in the production of the annual report and accounts, but we also welcome the views of your organisation. It is up to you which you give us. Your responses will be treated confidentially, and your details will not be passed to any other organisations. We collect contact information to allow us to contact respondents if we have follow-up questions. We are happy to receive more than one response from an organisation.

The survey should take less than 15 minutes to complete – only one question is required so feel free to complete as much or as little as you can. If a question is not applicable to you please skip that question.
