This survey is for parents and carers of children or young people between 0 to 25 years old with special educational needs and/or disabilities (SEND).
If you have more than one child with SEND you can use the same link to complete a separate survey for each child.
Welcome
Ofsted and the
Care Quality Commission (CQC) are visiting
Herefordshire Local Area Partnership to assess how effective the local area partnership’s commissioning arrangements are at identifying and meeting the needs of children and young people with SEND. Your views are important to us and will help inspectors understand your child's experience of the arrangements within your area.
Please watch the video to find out more about the inspection and how you can help.
The survey will remain open for your views until 9am on Tuesday 26 November 2024.
Accessibility
Use the 'Next page' and 'Previous page' buttons to navigate through the survey. Use the 'Finish survey' button at the end to submit your survey. Use the 'save and continue later' link if you would prefer to complete your response at another time.
If you have difficulties using this survey, please email
lasend.support@ofsted.gov.uk. You can ask someone to email for you.
Safeguarding concerns
If you think a child or young person is at risk of being abused or neglected, contact the children’s social care team at your local council. GOV.UK has further information about how to report child abuse to a local council. You can also contact the NSPCC on 0808 800 5000.
For young adults over the age of 18, local authorities have adult safeguarding teams. If you are concerned about a young adult, call your local council and ask to speak to the adult safeguarding team or look on the local adult safeguarding partnership website where the details of who to contact should be available.
We will treat the information you give us in confidence. We do not ask for your name, but we may try to identify you if you share something that makes us concerned about you or another person.
Safeguarding disclosures will be passed on to the local council and health services.
