About this survey
Every three years, Health and Wellbeing Boards are required to publish a Pharmaceutical Needs Assessment (PNA). The PNA is intended to identify what pharmaceutical services are currently available in each area and where services are likely to be needed in the future.
The information provided by you in this questionnaire will support the PNA being developed across Bristol, North Somerset and South Gloucestershire and help the Health and Wellbeing Boards understand local people’s experiences of accessing pharmacies.
We also ask for some information about you. This helps us to see if people in different areas and communities have different views and experiences that need to be better understood by the people making decisions.
By taking part in this survey, you agree that the comments that you provide can be used as part of a report shared with service providers and the public. We will not be able to identify who you are from your answers.
If you choose to leave contact details at the end of this survey, these will be stored in accordance with the Data Protection Act (2018). You can withdraw this permission at any time.
