Who is this form for
- substance use treatment providers
- criminal justice, housing, health including general practice, social care, and further education colleagues
- concerned peers, families and friends of the person experiencing a near-fatal incident
Our aim
Gathering information on drug related deaths contributes to our sudden death real-time surveillance system (RTSS) which helps us to identify clusters and trends early on and work with our local partners to help prevent more incidents.
The information also helps with the drug-related death review process. In these reviews, public health teams and other service providers look at the time leading up to the death. They check for any lessons to learn, like missed chances or good practices that can be shared across the local area.
