The Housing Support Grant Team is going to review the service you receive. The Housing Support Grant pay the costs for the housing related support which you receive and is also responsible for making sure that the service is helping you to achieve positive outcomes.
We would like to invite you to complete this questionnaire which seeks your views in relation to the support service you are receiving. Consultation will also involve undertaking a visit to your support provider to check the work undertaken by the project.
The questionnaire will be confidential. The exception to this would be if any potential risk to yourself or someone else was identified. All comments will be collated to inform the review process and may be used within the final report.
