Please complete this form to nominate yourself or a young person from you club on to the East Ayrshire Junior Sports Council. This will only take 5 minutes to complete and following the nomination the East Ayrshire Sports Council Secretary will be in contact.

Anyone being nominated must be between the ages of 11 - 25.
